The Prismatic chest for the Facet of Grace Fragment is in the Lost City region ofDestiny 2 The Final Shape’s Pale Heart, which you walk through during the very first mission of the campaign. However, you won’t be able to open the chest right away because you need a key, and standing next to the chest triggers the puzzle that gets you that key, so it’s important to know where to look:
How to get the Elevated Prismatic Key in Destiny 2

Upon approaching this chest, a bunch of Dread enemies will spawn, and you’ll see the message “Omens appear”. This is the start of the Dread-killing challenge that leads you to getting the Elevated Prismatic Key, so kill the Omen miniboss and their Dread minions, then be prepared to fight several more squads of Dread and eventually aDestiny 2 Tormentorboss. Here’s how you go about finding those Dread squads:
